Q: How does the dual-cure mechanism work in comparison to light-cure?

A: Dual-cure and light-cure mechanisms refer to the methods used to initiate the polymerization (hardening) of composite materials. In dual-cure composites, polymerization is initiated by both a chemical reaction (when the base and catalyst are mixed) and optional light activation. Light activation requires a dental curing light along with a light activated photo initiator. Depth of cure is typically 2-3 mm in materials that rely solely on light activation.

Q: What is Bio-Adaptive™?

A: Bio-Adaptive™ describes how Evanesce Bulk Cure’s self levelling, flowable viscosity and dual-cure formulation promotes superior marginal adaptation by sealing both dentin and enamel, regardless of cavity depth or irregularity, minimizing the risk of microleakage and gap formation.

Q: Is light-curing required to polymerize Evanesce Bulk Cure and, if so, are there specific light-curing times and intensities recommended?

A: With Evanesce Bulk Cure, light-curing is optional. To minimize shrinkage stress, we recommend waiting 60 seconds prior to light-cure polymerization. This allows the Bio-Adaptive™ properties, which are reliant on the self-cure polymerization process to promote marginal adaptation, encouraging sealing of both dentin and enamel to minimize the risk of microleakage and gap formation.

If light-curing after 60 seconds is desired, the recommendation is to use a dental curing light with the intensity of ≥1000mW/cm

Q: What is the maximum depth of cure for Evanesce Bulk Cure?

A: Evanesce Bulk Cure has an unlimited depth of cure and can achieve complete polymerization through its self-cure mechanism.

In contrast, the depth of cure of light-cured composite resins can vary based on the material and the performance of the dental curing light. Polymerization depths for light-cure materials is limited to 2-3 mm in depth with perfect conditions. There are some light-cured materials that state 4-5 mm depth of cure (bulk fills).

Q: What is the working time of Evanesce Bulk Cure, and does depth of cure effect that time?

A: For intraoral conditions, Evanesce Bulk Cure has a working time of approximately 30 seconds when used in a self-cure application (without curing light). The depth of the restoration does not affect the working time.

Q: What are the handling characteristics of Evanesce Bulk Cure? And how does it compare to a light-cure composite in terms of viscosity, flow, and ease of placement?

A: Evanesce Bulk Cure is a flowable bulk fill composite which provides control during placement yet is self levelling giving it Bio-Adaptive™ properties. Regardless of the cavity configuration, Evanesce Bulk Cure completely fills any cavity, creates an intimate adaptation to all adhesive walls, and a reduced chance of air entrapment.

Light-cure composites are pastes with varying viscosities. Common to all, is a higher chance of air entrapment during placement.

Q: Is Evanesce Bulk Cure compatible with all adhesive procedures? Is there a difference in the adhesive placement protocol compared to a light-cure composite?

A: Evanesce Bulk Cure is compatible with all dental adhesive procedures. There is no difference in adhesive placement techniques to consider vs. light-cure materials. Note: Dual-cure materials with amine/BPO as initiators may not be compatible with today’s universal, one-bottle adhesives. This is NOT a concern when using Evanesce Bulk Cure.

Q: Is a layer of adhesive required when capping Evanesce Bulk Cure?

A: No. If a capping layer is preferred vs a cut-back technique, Evanesce Bulk Cure does not require an additional layer of adhesive to bond the added layer of composite, which is the same as placing incremental layers using a light-cure composite.

If more composite is required after finishing Evanesce Bulk Cure, an additional adhesive layer would be required since the oxygen inhibitive layer will have been removed.

Q: Does Evanesce Bulk Cure provide the same esthetic results as a light-cure composite?

A: Categorized as a nanohybrid, Evanesce Bulk Cure provides excellent esthetic results while also improving mechanical properties (i.e., strength, adaptation, and wear-resistance). The formulation, comprised of sub-micron fillers and nano fillers, provides an optimal combination for posterior restorations.

Q: What are the differences in shrinkage rates and polymerization stresses between dual-cure and light-cure systems? How does a dual-cure system handle stress management in deep cavities?

A: Dual-cure materials undergo slower chemical polymerization before light activation, allowing for delayed gelation and stress relaxation, which reduces shrinkage stress (Hughes Oper Dent 2019). This minimizes issues like marginal failure, post-operative sensitivity, microleakage, secondary caries, and tooth fracture, which are common with light-cured composites.

With light-cure systems, increment thickness and curing time/intensity essentially maximize shrinkage forces and stress development which increases the chance of post-operative sensitivity and other clinical failures.

Q: How does the use of Evanesce Bulk Cure affect the overall efficiency of a procedure or chair time?

A: Evanesce Bulk Cure sets within 60 seconds in intraoral conditions. As a dual-cure composite, Evanesce Bulk Cure offers faster placement than traditional multi-increment light-cure composites due to its consistent flowability and ability to achieve both esthetic and mechanical properties in one step.

Evanesce Bulk Cure offers an unlimited depth of cure, allowing the entire preparation to be filled in a single increment without the need for light-curing. Large restorations (i.e., 6+ mm) require a minimum of two increments when using a light-cure bulk fill composite. When using a traditional 2 mm light-cure composite, three increments or more would be indicated.

The efficiency of Evanesce Bulk Cure meets or exceeds the efficiency of light-cure composite materials.

Q: What are the recommended indications for Evanesce Bulk Cure?

A: Evanesce Bulk Cure is indicated for the direct restoration of Class I, II, III, and V cavities, as a base or liner, and for core build ups. It is ideal for applications where light transmission is inadequate. Evanesce Bulk Cure is not recommended for use in anterior direct restorative procedures. In esthetic anterior cases we recommend Evanesce® or Evanesce Silk® Nano-enhanced Restorative Materials.
