Advantage+ Rewards Program
Terms and Conditions
1. Program Overview
The CRD Advantage+ Rewards program (the “Program”) is made available by Clinical Research Dental Supplies and Services Inc. (“CRD,” “we,” or “us”) to eligible customers (“Members”). The Program allows Members to earn and redeem points (“Points”) on qualifying purchases and program actions, and to unlock additional benefits based on tier status. By enrolling in or using the Program, the Member agrees to be bound by these Terms and Conditions (these “Terms”), CRD's standard terms of sale, and CRD's Privacy Policy.
The Program is a loyalty and discount program. Participation is permitted only in accordance with these Terms. CRD may update these Terms from time to time as described in Section 9.
2. Eligibility and Enrollment
The Program is open to dental private practices and dental group practices in Canada that maintain an active CRD online ordering account. CRD reserves the right to determine eligibility in its sole discretion.
Practices that are part of a Dental Service Organization (DSO), or that have any pre-existing negotiated price contract with CRD, are not eligible to enroll in or participate in the Program. A practice that becomes DSO-affiliated or enters into such a pricing contract after enrolling must notify CRD, and its participation will be discontinued as of the date the affiliation or contract takes effect; Points already earned up to that date will be governed by Section 9.
A practice must first have an active CRD online ordering account, then enroll in the Program and agree to these Terms before Points begin to accrue.
Locations that share a common CRD billing account number are enrolled together automatically. A new location added to an enrolled practice will be auto-enrolled within 24 hours. Practices with multiple billing accounts should contact CRD Customer Service to coordinate enrollment across locations.
Enrollment is free. There is no enrollment fee or annual membership fee to participate in the Program.
A Member's account is typically activated within 2 business days of enrollment confirmation.
The administrator on a Member's CRD online account is automatically designated the default administrator for that Member's CRD Advantage+ account. Administrators may view point balances, manage redemptions, and approve additional users as administrators. Additional users may request administrator access through the current administrator or the practice's Clinical Advisor.
3. Earning Points
Once enrolled, Members earn Points on virtually all CRD purchases at a rate determined by the Member's current tier (see Section 5), as well as through the program actions described below. CRD may add, remove, or change qualifying actions at any time.

Points are earned regardless of how an order is placed (online, by phone, or through a Clinical Advisor).
Points are earned on financed purchases in the same manner as other purchases.
Points continue to accrue on eligible purchases even if a Member's CRD account has an outstanding past due balance.
Certain products, services, and transaction types are excluded from earning Points. See Section 8 (Program Exclusions).
From time to time, CRD may offer Bonus Points promotions allowing Members to earn additional Points on selected products or collections for a limited time. Bonus Points offers are governed by these Terms and any additional terms posted with the specific offer.
4. Points Value and Redemption
Each Point carries a redemption value of $0.005 CAD (one half of one cent). For example, 1,000 Points equal $5.00 CAD in reward value and 10,000 Points equal $50.00 CAD.
Points may be redeemed for product vouchers, CE course vouchers, free shipping, and select physical products, as made available on the CRD Advantage+ Rewards page from time to time. Availability of specific rewards is not guaranteed and may change without notice.
A Member may begin redeeming Points once their practice reaches Insider tier status ($10,000 or more in qualifying purchases during the prior calendar year, as described in Section 5). Members at Access tier continue to earn Points but cannot redeem until Insider tier is reached.
There is no minimum purchase amount required to earn Points, and no limit on the number of Points that may be redeemed in a single transaction.
Points have no cash value, cannot be exchanged for cash, and cannot be sold, transferred, or assigned to another practice, account, or individual, except as CRD may expressly permit.
Once a redemption is confirmed, the corresponding Points are deducted from the Member's balance immediately. Confirmed redemptions cannot be self-service cancelled; a Member seeking to reverse a redemption must contact Customer Service, and any reversal is at CRD's discretion.
Physical product rewards are subject to availability. If a reward becomes unavailable after a Member has begun a redemption but before it is confirmed, no Points will be deducted.
5. Tier Status
A Member's tier status is based on the practice's total qualifying purchases during the previous full calendar year (January 1 through December 31) and is recalculated once per year, in January, based on the prior year's purchases.

Members are notified by email whenever their tier status changes. It may take up to 5 business days for new tier benefits to be reflected in the Member's account.
Tier-specific benefits (including discounts on select brands and early access to promotions, where applicable) are described on the Member's CRD Advantage+ dashboard and may be updated by CRD from time to time.
If a practice's qualifying purchases decrease, its tier may be adjusted downward at the next annual review. CRD may, at its discretion, apply a grace period before a downward tier adjustment takes effect; any such grace period will be described on the Member's dashboard or communicated by CRD.
6. Points Expiration
Points earned while a Member holds Premier or VIP tier status expire 2 years after the date they were issued.
Points earned while a Member holds Insider tier status expire 1 year after the date they were issued.
Points are tracked and expire on a first-earned, first-expired basis; a partial balance may expire while the remainder stays active.
If a Member's tier changes, Points already earned retain the expiration rule that applied when they were earned; expiration dates are not retroactively changed.
Members practicing in Quebec should refer to the Quebec-specific provisions below, or contact Customer Service, for any additional rules that may apply to Points expiration in that province.
7. Account Administration and Communications
CRD will send Members email notifications regarding enrollment, tier changes, Points expiring within 30 days, and material changes to the Program.
Members are responsible for keeping their contact information current to ensure timely receipt of Program communications.
A Member's CRD Advantage+ points balance, tier status, and full activity history are available at any time through the Member's CRD online account using single sign-on; no separate CRD Advantage+ login is required.
8. Program Exclusions
The following are examples of items and situations that may be excluded from earning or redeeming Points. This list is illustrative and may be updated by CRD from time to time; the current exclusion list will be made available to Members on request or on the CRD Advantage+ dashboard.
Shipping, freight, duties, and taxes
Gift cards and previously issued CRD Advantage+ vouchers
Certain third-party, special-order, or discontinued products as designated by CRD
Returned, cancelled, or charged-back orders (Points associated with the returned amount will be reversed)
Accounts that are suspended, closed, or found to be in violation of these Terms
Purchases made by, or attributable to, a practice that is part of a Dental Service Organization (DSO) or that has a pre-existing negotiated price contract with CRD
9. Program Changes, Suspension, and Termination
CRD may modify, suspend, or discontinue the Program, or any part of it, including tier thresholds, point values, earning rates, rewards, and exclusions, at any time and without prior notice, subject to applicable law.
CRD will make reasonable efforts to notify Members of material changes to these Terms in advance where required by law.
CRD may suspend or terminate a Member's participation in the Program, including forfeiture of accumulated Points, if the Member breaches these Terms, misuses the Program, or provides false information in connection with enrollment or redemption.
If the Program is discontinued, CRD will provide Members a reasonable opportunity to redeem accumulated Points before the discontinuation takes effect, except where the Program is discontinued for reasons of fraud, abuse, or legal or regulatory requirement.
10. Fraud, Abuse, and Misuse
CRD reserves the right to review any account for suspected fraud, abuse, or manipulation of the Program (including but not limited to artificially inflating purchases to trigger tier changes, or the improper transfer of Points between accounts). CRD may withhold, reverse, or cancel Points, redemptions, or tier status obtained through any such conduct, and may suspend or terminate the associated account.
11. Limitation of Liability
To the maximum extent permitted by law, CRD is not liable for any indirect, incidental, or consequential damages arising from a Member's participation in the Program, including losses relating to unavailable rewards, Points expiration, or Program changes. Nothing in these Terms limits any liability that cannot be limited under applicable law.
12. Privacy
Personal and practice information collected in connection with the Program is handled in accordance with CRD's Privacy Policy, available on the CRD website. By enrolling, Members consent to CRD's collection, use, and disclosure of information as described in that policy for purposes of administering the Program.
13. Governing Law
These Terms are governed by the laws of the applicable Canadian province or territory in which the Member's practice is located, without regard to conflict-of-law principles, and the federal laws of Canada applicable therein.
